As the Mental Health and Wellness Facilitator, I provide comprehensive support to strengthen student and staff well-being, increase school engagement, and foster collaborative relationships between families, staff, and community partners. My role centers on proactive intervention, relationship-building, and coordinating targeted support systems that meet the academic, behavioral, and social-emotional needs of students.

To support our students and staff, some of my key responsibilities include:

1. CARES Team Leadership

  • Lead and coordinate the school’s CARES Team to ensure students exhibiting academic, behavioral, or social-emotional needs receive timely, data-driven interventions.
  • Facilitate meetings, track student progress, and collaborate with teachers, counselors, and administrators to implement effective action plans.

2. Chronic Absenteeism Monitoring & Attendance Initiatives

  • Track and analyze attendance data to identify chronically absent or at-risk students.
  • Develop monthly attendance initiatives to celebrate and encourage students who meet the 90% monthly attendance threshold.
  • Partner with families, teachers, and support staff to remove barriers to attendance and improve student engagement.

3. New Student Orientation

  • Plan and deliver monthly new student orientation sessions to support smooth transitions for newly enrolled students.
  • Provide students and families with essential school information, expectations, resources, and connections to support services.

4. Community Partnership Coordination

  • Oversee current community partnerships to ensure ongoing communication, alignment to school needs, and meaningful service delivery.
  • Actively seek and cultivate new partnerships that provide enrichment, mental health support, family engagement, and resource access.

5. PTO & School Event Support

  • Collaborate with the Parent-Teacher Organization to plan and implement afterschool events that promote school culture, wellness, and family involvement.
  • Assist with logistics, communication, and event execution.

6. Staff Support & Professional Mentorship

  • Serve as the HEART Mentor for new teachers, providing guidance, support, and professional encouragement throughout their first year.
  • Assist educators in accessing resources, implementing wellness-focused strategies, and building a positive classroom culture.

7. Hospital Homebound Coordination

  • Manage and coordinate services for students receiving hospital homebound instruction.
  • Maintain communication between families, healthcare providers, and teachers to ensure continuity of instruction and compliance with district guidelines.

8. Family Support & Resource Navigation

  • Partner with the school social worker to connect families with essential resources including mental health services, housing supports, food assistance, and community programs.
  • Maintain strong, empathetic communication with families to ensure they feel supported and informed.

9. Schoolwide Mentor/Mentee Program

  • Lead the development and implementation of a schoolwide Mentor/Mentee Program designed to ensure every student is connected to a trusted caring adult in the building.
  • Coordinate the matching process so each student is paired with a staff mentor who will provide ongoing support, encouragement, and positive relationship-building throughout the school year.
  • Establish a bi-monthly meeting structure where mentors and mentees engage in check-ins focused on academic progress, attendance, social-emotional wellness, goal-setting, and celebrating student success.

10. Development of Schoolwide Peer Mentor Program (Upcoming Initiative)

  • Lead the design and implementation of a new Peer Mentor Program aimed at strengthening student leadership, social-emotional growth, and school connectedness.
  • Identify, train, and support student mentors who will serve as positive role models for their peers.
  • Collaborate with teachers, administrators, and support staff to create program structure, expectations, and curriculum.
  • Facilitate mentor/mentee matching, ongoing check-ins, and progress monitoring to ensure meaningful and productive relationships.
  • Integrate SEL components, conflict resolution strategies, and leadership skill-building to promote a supportive and inclusive school culture.
  • Collect qualitative and quantitative data to evaluate program effectiveness, make adjustments, and celebrate student successes.
